Sworn statement: what it should include
A sworn statement, or declaration, is a written statement in which you certify that facts are true: that you don't receive child support, that a relative lives with you, that you lost a document… It should give your full identity (name, date and place of birth, address), the facts stated precisely, the place, the date and your signature. For federal matters, 28 U.S.C. § 1746 lets you use an unsworn declaration signed "under penalty of perjury" in place of a notarized affidavit, and the template uses that wording; many states have a similar rule. Some agencies, courts and banks still require a notarized affidavit: in that case, print the document and sign it in front of a notary public. The proof of residence letter and the authorization letter follow the same logic, with the identity of the person who lives with you or the person you're authorizing.
Canceling internet, insurance, a subscription or a lease
For internet, cable or a cell phone plan, most providers let you cancel by phone or online, but a letter leaves a written record; if you're still under contract, an early termination fee may apply. Auto and home insurance can usually be canceled at any time, with a refund of the unearned premium, but make sure your new coverage starts first. For subscriptions and gym memberships, check the notice period in your contract; many states have automatic renewal laws that make canceling easier. To move out of a rental, the notice period depends on your lease and state law, often 30 days for a month-to-month tenancy. Give notice in writing, the way your lease requires, and keep proof of the date your landlord received it.
Demand letter, resignation, refund: the right letter
When a seller doesn't deliver or a product is defective, a demand letter sets a clear deadline and prepares the next steps: a chargeback with your card issuer, a complaint to your state attorney general's consumer protection office, or small claims court. Sent by certified mail with a return receipt, it proves the date of your request. To resign, most US jobs are at will and no particular form is required, but a dated letter avoids any dispute, and two weeks' notice is customary. For a refund, quote the seller's return policy, and if you paid by credit card, remember that you can dispute a billing error with your card issuer. Should I send my letter by certified mail?
Not always, but certified mail with a return receipt is the best proof of the date your letter was received. Check your lease for how a notice to vacate must be delivered. For a cancellation or a demand letter, certified mail heads off any dispute; for a resignation, handing it to your manager or emailing it is usually enough.
Does a sworn statement need to be notarized?
Not always. For federal matters, an unsworn declaration signed under penalty of perjury (28 U.S.C. § 1746) is accepted in place of a notarized affidavit, and many states have similar rules. But many agencies, banks and courts ask for a notarized affidavit: in that case, don't sign it until you're in front of the notary.
How much notice do I need to give my landlord?
